*This is the Creations Aerial Arts Competition/Showcase TEAM sign up. 
Team is by invite only.

Please create a MindBody account below, and log in to MindBody via app when you're ready to purchase your membership. TEAM- Your Coach/Miss Carissa will assist TEAM with this step. 
Please download the MindBody App on your phone and sign in from your phone then purchase your membership under "contracts".
​You will use the MindBody App on your phone to reserve your spot in class and sign up for classes.
*You must be age 18+ to purchase a membership.

Parents/guardians please create an account for yourself and add your child/children as members on your account so they can be signed in.

All parents/guardians must sign a waiver before their student can begin classes.
All parents/guardians MUST come in to drop off and to pick up their minor child regardless of age. 
If your child is old enough to drive, we will need a note from you.
 
If you sign into a class on MindBody and do not show, there is a $10 no show fee.
If you sign into a class on MindBody, and do not cancel within the allotted time specified, there is a $10 late cancellation fee.
You MUST SIGN IN TO YOUR CLASS 6 HOURS PRIOR TO CLASS STARTING. If you wait past 6 hrs to sign in, and you cannot sign up, you may try to call: 860.877.0074 but it is not guaranteed that you'll be let into that class. 
If you sign into a class on MindBody and do not show up for class your pass will NOT be put back onto your account unless you cancel within the allotted time frame specified on MindBody. 
*We have a strict no refund policy. 
Holidays are built into package pricing. 

"Additional Passes" are to be utilized ONLY if you have exceeded the # of passes you have that month and would like to add more class passes.

You will be billed each month on the date that you sign up for your membership.
*If at any time you wish to cancel your membership you MUST inform us in writing 30 days prior to your next billing cycle.
